Lead Hazard Control Projects Print

Contractors wishing to work on lead hazard control projects must meet the following qualifiations:

The company is registered with the State of Nebraska - sole proprietorship, LLC, Corporation.  Provide copy of State documentation.

The company is registered with Nebraska Workforce Development.  Provide copy of certificate.

Has the following insurance.  Provide copy of insurance certificate(s).

  • General liability - $2,000,000
  • Workmen's compensation

Please fill out and return the completed Annual Contractor Information Form.

Must be bondable (i.e., the contractor needs to show evidence that he/she can obtain a performance bond, if needed)

Must provide a list of three (3) references where:

  • the contractor has completed lead abatement work
  • the City can interview clients about project outcomes (please provide addresses and contact information)
  • the City can review the quality of work

Lead Training/Qualifications.  Provide copy of certifications or licenses.

  • EPA - RRP Certified Firm - Contractor and all employees trained
  • Lead Abatement Firm - At least one Lead Supervisor and all workers trained as Lead Workers

Please fill out and return the completed Human Rights & Relations Contract Compliance form (CC-1).

Please fill out and return the Citizenship Attestation Form for individuals or sole proprietorships.

The contractor is responsible for providing supporting documentation and references.  Submit all documentation to:
