Sidewalk Issues

Provide the location and information to describe the area of concern in the space below: *

• The house or commercial property address, or
• The street intersection and identify the specific corner (northeast, southeast, northwest or southwest), or
  

  (Example: 10th and Dodge Streets, northwest corner, icy sidewalk)

• If there are several properties in one block, provide the name of the street and identify the cross streets at each end of the block. Also, indicate whether the sidewalk is on the north, south, east, or west side of the street.
